Get to know Assessments

Assessments allows you to administer, score, and interpret validated clinical assessments directly within your patient session, improving efficiency, care quality, and the patient experience.

  • Guide patients through evidence-based assessments for objective screening, monitoring, and measurement of specific conditions

  • Run assessments during patient visits, and track progress in one streamlined workflow

  • View results instantly to inform accurate clinical decision-making

  • Send standardized assessments to patients securely, and document results easily in your EHR

Includes four ready-to-use assessments:

  • PHQ-9 (Patient Health Questionnaire with the self harm question)

  • PHQ-8 (Patient Health Questionnaire without the self harm question)

  • GAD-7 (Generalized Anxiety Disorder)

  • AUDIT (Alcohol Use Disorders Identification Test)

Both providers and patients can download the completed assessment for their records.


How to add Assessments to doxy.me

The first time you use Assessments on doxy.me, you’ll need to add the app to your account.

  1. Begin by signing into your doxy.me account

  2. On the dashboard, navigate to the Apps button on the Sidebar

  3. Now, find Assessments and click Add to doxy.me

  4. Once added, the app will be available to use


How to use Assessments during a session

Launch assessments anytime during an appointment from the Apps button on your Control Bar.

  1. Select an assessment
    Choose from supported, validated clinical instruments relevant to your specialty

  2. Administer assessment
    Securely share and administer assessments with the patient during the visit

  3. Automatic scoring and interpretation
    Assessments are scored in real time, with results immediately available

  4. Review and track outcomes
    Use results to inform care decisions and monitor patient progress over time


How to share Assessments with your patient

  1. Choose an available assessment and click Start to automatically share it with your patient

  2. You and your patient can work through the form together or independently

  3. Once all required fields are complete, the score will automatically calculate

  4. Click Download PDF

  5. Click Close Assessments to return to your appointment

💡 Once a provider starts the assessment, it's instantly shared with the patient


Available Assessments

Four standardized and evidence-backed assessments are included:

  • Patient Health Questionnaire-9 (PHQ-9) - For screening, diagnosing, monitoring and measuring the severity of depression, including the suicide question

  • Patient Health Questionnaire-8 (PHQ-8) - For screening and measuring the severity of depression, excluding the suicide question

  • Generalized Anxiety Disorder-7 (GAD-7) - For identifying and assessing the severity of generalized anxiety disorder

  • Alcohol Use Disorders Identification Test (AUDIT) - For assessing alcohol consumption, drinking behaviors, and alcohol-related problems


Understand the patient’s Assessments experience

The patient’s Assessments experience is similar to yours. When an assessment is shared, it will be displayed on their screen, while their video will minimize in the top right hand corner. Patients can scroll through the assessment at their own pace and will have the option to download for their own records.
