Get to know Document Builder

Document Builder allows you to upload and configure your own Teleconsent documents while preserving the original structure, branding, and legal language.

Many clinics rely on legally reviewed forms that must appear exactly as approved.

Document Builder ensures these documents are displayed and signed exactly as designed, including formatting, logos, page layout, and version markings.

You can use Document Builder to upload and manage documents such as:

  • Intake forms

  • Consent forms

  • Release of Information (ROI) forms

  • Authorizations

  • Other compliance-required documents

Once published, these documents become available within the Teleconsent app, for you to share with patients during visits.

💡 Document Builder allows you to upload and configure existing documents. It does not support creating new forms from scratch


How to start using Document Builder

The first time you use Document Builder on doxy.me, you’ll need to add the Teleconsent app to your account and agree to the Terms of Service.

  1. Begin by signing into your doxy.me account

  2. On the Dashboard, navigate to the Apps button on the Sidebar

  3. Now, find Teleconsent and click Add to doxy.me

  4. When adding Teleconsent, you’ll be prompted to accept the Terms of Service

  5. Once accepted, the app will be available and you can start using Document Builder

How to create a document with Document Builder

  1. To use Document Builder, you must first upload and configure your documents within Teleconsent app settings

  2. Sign in to your doxy.me account

  3. From your Dashboard, go to Settings

  4. Select Apps, then select Teleconsent

  5. Choose Create

  6. Upload your approved form. This must be in PDF format.

    1. Either drag and drop a file or upload a file from your device

    2. Your uploaded PDF will appear and you can begin to customise

How to customize a document with Document Builder

  1. Customization options:

    1. Name your document. This will auto-populate from the title of your uploaded file, but you can edit this as needed

    2. Each block can be assigned to you, your patient, or both you and the patient - so everyone knows exactly what they’re responsible for completing

    3. Click on any customizable content blocks and add to your document including signature, dates, text field and drop downs options

    4. Change height and length of the custom fields as needed

    5. Scroll through multiple pages and navigate between them to add content blocks throughout your entire document

  2. While editing, or once you’re finished, you can preview your document using the toggle in the top right corner

  3. Once your document is final, click Publish

  4. Your custom document will appear at the top of the list of available Teleconsent forms within your app settings

  5. Once published, the document will be available to any providers in your clinic tp use during Teleconsent sessions

Note: Once created, you cannot edit a custom document, You would need to archive or delete this and create another.

💡 Only Owners and Admins can upload or manage custom documents


How to use Document Builder documents during a session

After a document has been published, providers can share it with patients during a Teleconsent session.

  1. Start your doxy.me appointment

  2. From the Control Bar, select Apps

  3. Choose Teleconsent

  4. Select the document you want to share

  5. Click Start share to begin reviewing the document with your patient

💡 You and your patient can review the document together, complete required fields, and digitally sign it


Completing and saving documents

Once all required fields are completed:

  1. Click Generate PDF

  2. Both you and your patient will be prompted to download the finalized document

  3. Save the document to your device or EHR system

  4. Click Close Teleconsent to return to the appointment


Archiving and restoring documents

Owners and Admins can manage document availability in Teleconsent settings.

  • Archive a document to remove it from provider selection

  • Restore a document to make it available again during sessions

Archived documents are not visible to providers during Teleconsent sessions.


Understand the patient’s Teleconsent experience

When a document is shared, it appears directly on the patient’s screen while their video minimizes to the top corner.

Patients can:

  • Scroll through the document at their own pace

  • Fill in fields assigned to them

  • Review highlighted sections

  • Provide digital signatures

💡 Each field is designated for either the provider or patient, ensuring the correct person completes each section
